LOS ANGELES, Calif. -- Natalie Cole has been released from the hospital in New York and is now back home, Access Hollywood has confirmed.

The legendary songstress was hospitalized last month after falling ill due to the effects of chemotherapy, which was being used to combat liver disease from Hepatitis C.

But after two weeks in the hospital, Cole is finally back home -- however, her treatment continues.
